| The Senior Transition Program provides specialized services to persons over the age of
50 with a variety of abilities, disabilities, and aging characteristics. Senior
participants are involved in the design of their activities and have a variety of options
from which to choose. The program emphasizes a holistic approach which builds on
individual strengths to foster self-esteem and remotivation. A consistent emotional
climate and stable environment support participants in transition through the life changes
and losses often associated with aging. Program staff are skilled in group process which
includes coaching a healthy, appropriate interdependence among group members. Key
components of the Senior Transition Program are the Daily Life Program, Advocacy
and Community Integration.

Day Activity Program
|
A day
program of structured activities operates from 9:00 a.m. - 3:00 p.m.,
Monday through Friday. The schedule of activities includes reality
orientation, exercise, current events and reminiscence discussions, music,
bingo and other leisure games, sensory/motor activities, and cooking.
Activities most often are created to carry out themes of interest to the
group or to teach specific skills. Frequently activities relate to
outreach or charity projects planned by the group; these projects promote
self-esteem through service and fill a need for meaningful productivity.

Advocacy
|
Advocacy on an individual level is carried out by means of consistent, close
communication with families, care givers, and other service providers. By daily recording
of observational data, an ongoing assessment of each program participant is realized. The
analysis of this data provides rationale for needed services and health care for
individuals. Program personnel provide systems advocacy through formal presentations and
informal education opportunities with other service providers regarding aging issues and
the needs of persons who are visually impaired. |
Community Integration
|
Community exploration opportunities are provided for the entire
group through specific, planned activities and outings. On an individual basis,
integration is accomplished through training and support at community senior center sites
or volunteer job placements. These options are developed and implemented according to
individual choice. |
The Senior Transition Program has been in existence at the Oakland site of Blind and Vision Rehabilitation Services of Pittsburgh since 1991 with funding provided by Allegheny County MH/MR and licensing
as an Adult Day Services program by Pennsylvania Department of Aging.
